Are you passionate about helping individuals build a path to lasting recovery? SSTAR's Fall River Inpatient Program is seeking a dedicated and motivated Case Manager to join our team!
In this rewarding role, you'll play a vital part in supporting patients as they transition from inpatient care to continued treatment and community resources. You'll work closely with a collaborative, compassionate team to ensure each individual has the tools and connections they need for success beyond our program.
What You'll Do- Provide comprehensive case management services focused on aftercare planning
- Connect clients with ongoing treatment, housing, and community resources
- Collaborate with a multidisciplinary team to support patient progress
- Help foster a safe, supportive, and recovery-focused environment

Bilingual candidates of all genders are strongly encouraged to apply.
- Education: High school diploma or GED required; Bachelor's degree in Social Work or a related field preferred.
Strong verbal and written communication skills.
Open to working with people from diverse backgrounds, cultures and lifestyles in a non-judgmental manner.
Direct care positions in addiction programs require two years of continuous sobriety for individuals in recovery.
