Licensed Master Social Worker (LMSW) – Contract

Location: New York, NY
Schedule: Monday – Friday | 9:00 AM – 5:00 PM
Hours: 35 Hours per Week

Job Summary

"Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker (LMSW) to support patients within an inpatient medicine/surgery setting. The LMSW will provide psychosocial assessments, supportive counseling, discharge planning, and referral coordination to ensure patients receive comprehensive care and support throughout their hospitalization.

Key Responsibilities

• Conduct psychosocial assessments for inpatient medical and surgical patients
• Provide supportive counseling and therapeutic interventions
• Develop and coordinate discharge plans
• Make referrals to community resources and support services
• Collaborate with physicians, nurses, case managers, and interdisciplinary teams
• Document assessments, interventions, and patient progress accurately
• Advocate for patients and assist with care coordination needs
• Support continuity of care and transition planning

Required Qualifications

• Master's Degree in Social Work (MSW)
• Active New York State Licensed Master Social Worker (LMSW) license
• Minimum 1 year of LMSW experience
• Strong communication, counseling, and documentation skills
• Ability to work effectively in an inpatient hospital environment
