About this job

The purpose of the Permanent Supportive Housing Coordinator is to stay in close contact with individuals and families that are placed in Permanent Supportive Housing, making sure that they are fulfilling the requirements of the program, and remain housed.  This position is intended to coordinate with internal and external partners to house the homeless and prevent homelessness.

 

The PSH Coordinator will report to the Supportive Housing Manager, and is responsible to:

 


  • Maintain records on all of the individuals housed in the program

  • Coordinate with service providers to assist with case management

  • Work closely with the Housing Resource Coordinator and the NEST Director to serve individuals who have the potential to be sustainably housed

  • Assist with the management of the Food Bank and other services that provide supports to those who are housed and unhoused  

  • Develop support groups and opportunities for social connection among those in the program

  • Collect occupancy charges

  • Coordinate with community partners and providers to connect homeless individuals to needed services and resources for stabilization

  • Work closely with Housing Program Manager to develop Individual Success Plans

  • Other duties as assigned

 

QUALIFICATIONS


  • High School Diploma

  • Experience of at least two years working marginalized populations

  • Experience and proficiency with computer use

  • Ability to work as part of a team

  • Self-motivated with the ability to work independently

  • Ability to work with community partners and advocate

  • Ability to maintain confidentiality as it pertains to agency records

  • Understanding of and ability to work in a cross-cultural environment
