Key Responsibilities of the Pharmacist:
Review, verify, and dispense medications accurately in accordance with physician orders and clinical guidelines
Evaluate medication therapies for appropriateness, effectiveness, safety, and potential drug interactions
Collaborate with physicians, nurses, and healthcare teams to optimize patient medication management
Provide pharmacological consultations and evidence-based recommendations to healthcare providers
Counsel patients regarding medication use, side effects, and adherence strategies when appropriate
Monitor patient medication profiles and participate in medication reconciliation processes
Ensure compliance with state, federal, and facility pharmacy regulations and policies
Maintain accurate documentation and support quality improvement and patient safety initiatives
Requirements:
Minimum 2 years of recent Pharmacist experience in a hospital or clinical setting
Active Louisiana Pharmacist license or eligibility for licensure in Louisiana
Doctor of Pharmacy (PharmD) degree from an accredited program preferred
Strong knowledge of pharmacotherapy, medication safety, and clinical pharmacy practices
Excellent communication, analytical, and problem-solving skills
Ability to work variable day and evening shifts in a fast-paced healthcare environment
Experience with electronic medical records and pharmacy information systems preferred
