Requirements Locums |Valdez, AKRural Health________________________________________________________________SCHEDULE• Coverage Type:Inpatient• Schedule Type:Clinic & Call• Call Type:Overnight Beeper Call• Dates of Need:July 16th- August 10th and August 19-28th, October shifts also probable• Shift Times:10 hours 8a-6p, night call. ~4 10 hour shifts per week. • LunchN/A• Provider Min # Shifts Required: We need a provider who can cover as much as possible• Guaranteed Hours per Day:10• Hours Worked for OT to apply:10• Rates to be Filled Out on Bid:Clinic & Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.________________________________________________________________CREDENTIALING• Current CV (must be uploaded to bid)• License:Active or IMLC• DEA Required:Yes• Boards:BC only- our preference would be for providers Board Certified Family Practice, with rural health experience ( ED, Hospitalist, OB) experience. • Skills/Certs: Rural Health• Other:________________________________________________________________ADDITIONAL DETAILSProvidence Valdez is entering a limited window of time that we will be needing extra Provider support for our Critical Access Hospital.We are a small rural hospital, and cover broad scope but transfer patients out as necessary.The Providers would need to be willing and able to do ER, Outpatient/Primary Care, OB/C-Sections if needed in emergency.MSO will asses files and see how quickly they are able to issue privs- this is a high acuity need so all hands are on deck to help with coverage.Travel need to knows? Fly into anchorage, and take regional flight or ground shuttle into Valdez. Ground shuttle is about $600, regional flight should not exceed cost of shuttle. Lodging- the hospital has apartments. No pets.
