Geriatric Nurse Practitioner / Physician (MD/DO) | PT / FT

Join a growing healthcare team dedicated to providing high-quality primary care services to seniors in assisted living communities throughout New York's Capital Region. We are seeking an experienced Nurse Practitioner (NP) or Physician (MD/DO) who is passionate about geriatric care and building meaningful, long-term relationships with residents.

Schedule Options:

Part-Time

• 2–3 full days per week
• Travel between Hoosick Falls, NY and Cambridge, NY

Full-Time

• 4–5 full days per week
• Travel between Hoosick Falls, NY, Cambridge, NY, and Troy, NY

Required Qualifications

• Active New York State Nurse Practitioner or Medical Doctor (MD/DO) license
• Active DEA registration
• Current enrollment and credentialing with both Medicare and Medicaid
• Ability to practice fully independently
• NPs must meet New York State requirements including a minimum of 3,600 clinical practice hours
• Primary care experience in a related setting
• Reliable transportation and willingness to travel between outpatient clinics

 Preferred Qualifications

• Experience working with geriatric patients
• Assisted living, skilled nursing, or long-term care experience
• Strong communication and relationship-building skills
• Passion for delivering patient-centered care

Compensation & Benefits

• Starting at $60+ per hour (based on experience and credentials)
