Job Description
Licensed Practical Nurse (LPN) – Long-Term Care (LTC)
Location: Falmouth, MA 02540
Duration: 13 Weeks
Shift: Evening Shift
Hours: 3:00 PM – 11:30 PM
Schedule: 40–42.5 Hours Per Week | 5 Days Per Week
Weekend Requirement: Every Other Weekend
Holiday Requirement: Rotating Holidays
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join a Long-Term Care team in Falmouth, Massachusetts. The ideal candidate will be committed to delivering high-quality patient-centered care while promoting resident dignity, safety, and well-being.
Requirements:
Active, unencumbered Massachusetts LPN License.
Current BLS Certification required.
Current CPR Certification required.
Current AED Certification required.
Strong skills in medication administration and resident assessment.
Experience with electronic documentation and care plan implementation.
Knowledge of long-term care nursing practices and infection control standards.
Experience with wound care, catheter care, and tube feeding management.
Strong patient education, communication, and organizational skills.
Leadership and staff supervision experience preferred.
Responsibilities:
Provide direct nursing care to residents in accordance with physician orders and individualized care plans.
Administer medications and treatments accurately and safely.
Monitor resident conditions and report changes to physicians and nursing leadership.
Record and document vital signs, assessments, and resident responses to treatment.
Collect specimens such as urine, blood, and sputum for laboratory testing.
Perform skilled nursing procedures including catheterization, tube feedings, suctioning, wound care, dressing changes, colostomy care, drainage bag management, and range of motion exercises.
Supervise nursing assistants and support staff during assigned shifts.
Maintain accurate resident records and ensure compliance with facility policies and regulatory standards.
Educate residents and families regarding treatments, medications, and care plans.
Promote resident safety, dignity, privacy, and quality of care.
Collaborate with interdisciplinary teams to support positive resident outcomes.
