LPN – Long Term Care

Location: Falmouth, Massachusetts

Contract Duration: 13 Weeks

Compensation:

  • Travel Rate: $48–$51/hour ($1,920–$2,040/week) based on experience
  • Local Rate: $40–$45/hour ($1,600–$1,800/week) based on experience

Schedule:

  • 40 Hours per Week
  • Evening Shift: 3:00 PM – 11:30 PM
  • Every Other Weekend Required
  • Rotating Holidays
Position Overview

Seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join a Long-Term Care team dedicated to providing exceptional resident-centered care. The LPN will be responsible for delivering direct nursing care, supervising nursing assistants, administering medications and treatments, and ensuring compliance with all regulatory and facility standards.

Requirements
  • Current, active, unencumbered Massachusetts LPN License
  • Graduate of an approved LPN program
  • BLS/CPR/AED Certification required
  • Long-Term Care or Skilled Nursing experience preferred
  • Strong documentation and clinical assessment skills
  • Ability to lift up to 50 pounds and meet physical requirements of the position

Key Responsibilities
  • Provide direct nursing care to residents in accordance with physician orders and care plans
  • Supervise and direct nursing assistants during assigned shifts
  • Administer medications and treatments safely and accurately
  • Complete admissions, transfers, and discharge documentation
  • Monitor resident conditions and report changes to physicians and nursing leadership
  • Assist with care planning and interdisciplinary team collaboration
  • Maintain accurate and timely clinical documentation
  • Educate residents and family members regarding care and treatment plans
  • Ensure compliance with federal, state, and facility regulations
  • Participate in surveys, inspections, and quality improvement initiatives
