Licensed Practical Nurse (LPN) – Long-Term Care (LTC)

Location: Falmouth, MA 02540
Duration: 13 Weeks
Shift: Evening Shift
Hours: 3:00 PM – 11:30 PM
Schedule: 40–42.5 Hours Per Week | 5 Days Per Week
Weekend Requirement: Every Other Weekend
Holiday Requirement: Rotating Holidays

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join a Long-Term Care team in Falmouth, Massachusetts. The ideal candidate will be committed to delivering high-quality patient-centered care while promoting resident dignity, safety, and well-being.

Requirements:

  • Active, unencumbered Massachusetts LPN License.

  • Current BLS Certification required.

  • Current CPR Certification required.

  • Current AED Certification required.

  • Strong skills in medication administration and resident assessment.

  • Experience with electronic documentation and care plan implementation.

  • Knowledge of long-term care nursing practices and infection control standards.

  • Experience with wound care, catheter care, and tube feeding management.

  • Strong patient education, communication, and organizational skills.

  • Leadership and staff supervision experience preferred.

Responsibilities:

  • Provide direct nursing care to residents in accordance with physician orders and individualized care plans.

  • Administer medications and treatments accurately and safely.

  • Monitor resident conditions and report changes to physicians and nursing leadership.

  • Record and document vital signs, assessments, and resident responses to treatment.

  • Collect specimens such as urine, blood, and sputum for laboratory testing.

  • Perform skilled nursing procedures including catheterization, tube feedings, suctioning, wound care, dressing changes, colostomy care, drainage bag management, and range of motion exercises.

  • Supervise nursing assistants and support staff during assigned shifts.

  • Maintain accurate resident records and ensure compliance with facility policies and regulatory standards.

  • Educate residents and families regarding treatments, medications, and care plans.

  • Promote resident safety, dignity, privacy, and quality of care.

  • Collaborate with interdisciplinary teams to support positive resident outcomes.
