Job Title: Licensed Practical Nurse (LPN) – Long-Term Care
Location: Falmouth, MA 02540
Employment Type: Contract - 13 Weeks
Schedule- Shift: Evening
- Hours: 3:00 PM – 11:30 PM
- 40 hours per week
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality direct nursing care in a Long-Term Care (LTC) setting. The LPN will supervise nursing assistants, administer medications and treatments, and ensure accurate clinical documentation while maintaining compliance with all facility, state, and federal regulations.
Key Responsibilities- Provide direct nursing care in accordance with care plans and physician orders
- Supervise and guide nursing assistants during assigned shifts
- Administer medications and treatments safely and accurately
- Monitor resident conditions and document any changes
- Transcribe and implement physician orders and update care plans
- Perform clinical procedures including wound care, catheter care, tube feedings, suctioning, and colostomy care
- Collect specimens and perform basic lab testing as required
- Record vital signs including temperature, pulse, respiration, and blood pressure
- Maintain accurate, timely, and compliant documentation
- Assist with resident admissions, transfers, and discharges
- Collaborate with interdisciplinary healthcare teams
- Ensure confidentiality and compliance with HIPAA and resident rights
- Support audits, inspections, and regulatory surveys
- Maintain medication and supply inventory as needed
- Active, unrestricted LPN license (Massachusetts required or eligible)
- Graduate of an accredited LPN program
- Current BLS/CPR/AED Certification
- Strong knowledge of long-term care nursing practices
- Ability to communicate effectively in English (written and verbal)
- Ability to work independently and make sound clinical decisions
- Ability to meet physical demands, including lifting up to 50 lbs
- Previous experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF)
- Strong leadership and supervisory skills
- Experience with EMR systems
- Excellent patient care and communication skills
- Skilled Nursing / Long-Term Care facility
- Fast-paced, patient-centered environment
- Exposure to geriatric and high-acuity residents
- May require weekends and rotating holidays
