Job Title: Licensed Practical Nurse (LPN) – Long-Term Care

Location: Falmouth, MA 02540

Employment Type: Contract - 13 Weeks

• Shift: Evening
• Hours: 3:00 PM – 11:30 PM
• 40 hours per week

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality direct nursing care in a Long-Term Care (LTC) setting. The LPN will supervise nursing assistants, administer medications and treatments, and ensure accurate clinical documentation while maintaining compliance with all facility, state, and federal regulations.

• Provide direct nursing care in accordance with care plans and physician orders
• Supervise and guide nursing assistants during assigned shifts
• Administer medications and treatments safely and accurately
• Monitor resident conditions and document any changes
• Transcribe and implement physician orders and update care plans
• Perform clinical procedures including wound care, catheter care, tube feedings, suctioning, and colostomy care
• Collect specimens and perform basic lab testing as required
• Record vital signs including temperature, pulse, respiration, and blood pressure
• Maintain accurate, timely, and compliant documentation
• Assist with resident admissions, transfers, and discharges
• Collaborate with interdisciplinary healthcare teams
• Ensure confidentiality and compliance with HIPAA and resident rights
• Support audits, inspections, and regulatory surveys
• Maintain medication and supply inventory as needed

• Active, unrestricted LPN license (Massachusetts required or eligible)
• Graduate of an accredited LPN program
• Current BLS/CPR/AED Certification
• Strong knowledge of long-term care nursing practices
• Ability to communicate effectively in English (written and verbal)
• Ability to work independently and make sound clinical decisions
• Ability to meet physical demands, including lifting up to 50 lbs

• Previous experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF)
• Strong leadership and supervisory skills
• Experience with EMR systems
• Excellent patient care and communication skills

• Exposure to geriatric and high-acuity residents
• May require weekends and rotating holidays
