Licensed Practical Nurse (LPN) – Long-Term Care

Location: Falmouth, MA 02540
Duration: 13 Weeks
Shift: Varies by Facility Need
Hours: Full-Time

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide direct nursing care and support residents in a Long-Term Care setting. The ideal candidate will have strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality resident-centered care.

Requirements:

  • Graduate of an approved Licensed Practical Nursing (LPN) program.

  • Current, active, and unrestricted Massachusetts LPN license.

  • Current BLS/CPR/AED Certification required.

  • Knowledge of nursing practices, medical procedures, and long-term care regulations.

  • Ability to provide age-appropriate care to geriatric and long-term care populations.

  • Strong clinical assessment and documentation skills.

  • Ability to make independent clinical decisions when appropriate.

  • Ability to read, write, speak, and understand English.

Responsibilities:

  • Provide direct nursing care in accordance with physician orders, care plans, and nursing standards.

  • Perform nursing assessments and monitor resident conditions.

  • Administer medications and treatments as prescribed.

  • Review resident charts, physician orders, and treatment plans.

  • Collaborate with physicians and interdisciplinary teams regarding resident care.

  • Participate in care planning and physician rounds.

  • Collect laboratory specimens as needed.

  • Perform clinical procedures including wound care, catheter care, tube feeding, suctioning, and other nursing interventions.

  • Monitor and document vital signs and resident responses to treatment.

  • Maintain accurate documentation for admissions, transfers, discharges, and daily nursing care.

  • Process physician orders and maintain compliance with documentation standards.

  • Supervise Nursing Assistants and support staff during assigned shifts.

  • Coordinate daily resident care activities and assignments.

  • Educate residents and families regarding treatments, medications, and care plans.

  • Promote resident rights, dignity, privacy, and independence.

  • Ensure adequate medical supplies and equipment are available.

  • Participate in quality improvement initiatives, training, and departmental meetings.

  • Perform additional duties as assigned.
