Job Description
We are seeking a motivated and patient-focused Medical Assistant/Care Coordinator to support providers through a unique in-home telehealth care model. In this role, you will travel to patients' homes, facilitate tele-video visits, perform basic clinical duties, and coordinate follow-up care under the direction of the provider.
This position requires a combination of clinical skills, patient engagement, technology support, and care coordination. You will serve as the provider's eyes and ears in the home while helping patients receive high-quality care in a comfortable setting.
Key Responsibilities
- Travel to patient homes to facilitate scheduled tele-video visits with healthcare providers.
- Set up telehealth equipment and ensure successful connection between the patient and provider.
- Obtain and document patient vital signs, including blood pressure, pulse, oxygen saturation, weight, and temperature.
- Assess patient concerns and gather relevant information for the provider prior to and during the telehealth visit.
- Receive training to perform clinical duties, including:
- Phlebotomy (blood draws)
- Administration of injections
- Ear lavage procedures
- Collection of specimens as needed
- Accurately document all patient interactions, assessments, procedures, and provider instructions in the EMR.
- Coordinate and process laboratory orders, imaging studies, referrals, and follow-up appointments.
- Process medication refill requests and prescription orders under provider direction and established protocols.
- Receive training to place orders and coordinate care activities under provider supervision, including verbal directives when appropriate.
- Conduct weekly patient coordination with wound care and other teams to ensure no double booking occurs and follow-up visits are scheduled to assess progress, identify concerns, and support care plan adherence.
- Communicate effectively with patients, caregivers, pharmacies, providers, and healthcare partners.
- Assist in identifying barriers to care and escalating concerns to the provider when necessary.
Qualifications
Required
- Active Certified Medical Assistant (CMA) certification or equivalent clinical credential.
- Experience using Electronic Medical Records (EMR).
- Strong clinical assessment and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently in a field-based environment.
- Valid driver's license, reliable transportation, and willingness to travel throughout the service area.
- Experience with eClinicalWorks (eCW).
Preferred
- Experience in primary care, geriatrics, home-based care, or care coordination.
- Experience performing blood draws, injections, and ear lavages.
- Experience ordering labs, imaging, referrals, and coordinating follow-up care.
- Familiarity with telehealth platforms and remote patient care.
