Shift:
- Monday–Thursday: 8:00 AM – 6:00 PM
- Friday: 8:00 AM – 12:00 PM
We are seeking a skilled Physical Therapist to provide evaluation, treatment planning, and therapeutic care in accordance with physician referrals and regulatory standards. This role requires strong clinical expertise to deliver age-appropriate, individualized patient care in a collaborative healthcare environment.
Key Responsibilities
Patient Evaluation & Care Planning
- Evaluate patient functional abilities and limitations
- Develop individualized plans of care with measurable, functional goals
- Initiate discharge planning and recommend treatment modifications
Treatment Implementation
- Deliver safe, evidence-based therapy interventions
- Adapt plans based on patient progress
- Engage patients and caregivers in treatment and decision-making
Documentation & Compliance
- Maintain accurate, timely documentation aligned with regulatory standards
- Demonstrate skilled intervention and track patient outcomes
Communication & Collaboration
- Communicate patient status and progress with physicians, nurses, and care teams
- Provide ongoing education to patients, families, and staff
- Collaborate across interdisciplinary teams for optimal outcomes
Team & Operational Support
- Supervise and delegate to PTAs, technicians, and students as appropriate
- Participate in staff training, departmental meetings, and process improvements
- Meet productivity and quality standards
- Direct patient care setting
- Frequent standing, walking, bending, and lifting (up to 75 lbs)
- Exposure to standard healthcare precautions
- Requires strong coordination, mobility, and communication skills
- Graduate of an APTA-accredited Physical Therapy program (or equivalent)
- Active Arizona Physical Therapist license
- BLS certification
- Strong clinical competency and patient care experience
- Ability to use healthcare technology and documentation systems
Preferred Qualifications
- Previous travel or acute care experience
- Additional relevant clinical education or specialized training
