Territory: lav/purple/orange
**Must have at least 12 months of home health experience within the past 3 years
Must have HCHB and OASIS experience
Guaranteed 40 hours per week. Must meet minimum productivity expectation of 32 points per week.
DAYS -  8HRS: M - F 8 - 5
