University Home Care Inc is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in Southeast MI Region. Must be willing to travel. This role involves providing high-quality nursing care to patients in a home care setting, ensuring their health and comfort while promoting independence and well-being.

Responsibilities

• Provide direct nursing care to patients in their homes, following care plans and physician instructions.
• Monitor patient health and report any changes to the healthcare team.
• Administer medications, injections, and treatments as prescribed.
• Maintain accurate and timely documentation of patient care and progress.
• Educate patients and their families on health management and disease prevention.
• Collaborate with other healthcare professionals to ensure coordinated care.

Requirements

• Current LPN license valid in the state of Michigan.
• Previous experience in home health care or a related nursing field preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate and patient-centered approach to care.
• Reliable transportation to travel to patient homes in the Southeast MI Region.
