LPN Home Care

 

University Home Care Inc is seeking a dedicated and compassionate LPN Home Care professional to provide high-quality nursing care to patients in their homes in the Southeast MI Region. Must be willing to travel.  The ideal candidate will be responsible for delivering personalized care plans, monitoring patient health, and supporting overall wellness to ensure the best possible outcomes for our clients.

Responsibilities

• Provide skilled nursing care to patients in their home environments.
• Monitor and assess patient conditions, documenting changes and reporting to healthcare providers as necessary.
• Administer medications and treatments as prescribed.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans.
• Educate patients and families on health maintenance, disease prevention, and care management.
• Maintain accurate and timely documentation of nursing care and patient progress.
• Ensure compliance with home care regulations and company policies.

Requirements

• Valid Licensed Practical Nurse (LPN) license for the state of Michigan.
• Previous experience in home care or a similar healthcare setting preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and manage time efficiently.
• Compassionate and patient-centered approach to care.
• Reliable transportation to visit patients in the Southeast MI Region.
