Are you interested in playing a key role in protecting and assisting older adults in your community? Monroe County Area Agency on Aging is searching for an Aging Care Manager 3 to work with individuals, families, and community partners to ensure safety and well-being. This position allows you to build strong relationships while responding to important needs in the community, offering support, guidance, and coordination for essential services. DESCRIPTION OF WORK
As an Aging Care Manager 3, you will help protect older adults by reviewing reports of need, conducting investigations, and coordinating essential services. Your work supports safety, well-being, and strong community connections. In this role, you will perform the following duties:
- Report Review: Receive and evaluate reports of need in accordance with the Older Adult Protective Services Act
- Timely Investigation: Begin investigations promptly, complete required steps, and determine the level of risk to the older adult
- Care Planning: Develop care plans, arrange assessments, and document case details in the SAMS system
- Community Education: Provide outreach and public education on protective services and mandatory reporting requirements
- Case Management: Manage complex cases, coordinate resources, and maintain strong working relationships to reduce risk
- Emergency Response: Provide 24 hour emergency services as scheduled and support the Elder Abuse Task Force
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
- Travel, overtime, and after hour duties may be required.
- Telework: You may have the opportunity to work from home (telework) part-time, after six months of employment. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Stroudsburg. The ability to telework is subject to change at any time. Additional details may be provided during the interview.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.

- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
