Title: RN – Case Manager

📍 Location: Pocatello, Idaho
🕒 Shift: 5x8 Hour Days

Responsibilities

• Perform medical necessity and utilization reviews
\t
• Coordinate discharge planning and patient transitions of care
\t
• Identify and communicate changes in the patient's plan of care to the healthcare team
\t
• Maintain detailed knowledge of community resources and referral processes
\t
• Anticipate patient needs using LOS (Length of Stay) guidelines
\t
• Ensure stewardship of resources and timely transitions to alternative levels of care
\t
• Complete timely and accurate documentation, including:\t
  \t\t
  • Initial screenings
  \t\t
  • Discharge planning notes
  \t\t
  • Patient care coordination documentation
  \t
  \t

Requirements

\t
• RN Case Management experience preferred
\t
• Minimum 2 years of experience preferred
\t
• First-time travelers accepted
