The Community Care Coordinator serves as the bridge between residents, families, caregivers, and healthcare providers to ensure residents receive personalized, compassionate care while enjoying the highest quality of life possible.



This leadership role combines resident advocacy, care coordination, team support, and family communication. The ideal candidate is organized, compassionate, proactive, and passionate about serving older adults.

The Community Care Coordinator plays a critical role in creating a warm, supportive environment where residents thrive and families have confidence in the care their loved ones receive.



Essential Responsibilities

Resident Care Coordination

  • Coordinate and oversee resident care services across all shifts.
  • Monitor resident health and wellness needs and communicate changes appropriately.
  • Review resident records and ensure care plans remain current and accurate.
  • Coordinate physician appointments, home health services, hospice services, and other healthcare providers as needed.
  • Conduct routine resident assessments and follow-up evaluations.
  • Support residents in achieving the highest level of independence and well-being possible.
  • Ensure compliance with community policies, state regulations, and documentation requirements.

Family Communication & Resident Advocacy

  • Serve as a primary point of contact for residents and family members regarding care concerns and questions.
  • Build strong relationships with residents and their loved ones through regular communication and support.
  • Address concerns promptly, professionally, and compassionately.
  • Advocate for resident needs while maintaining dignity, choice, and independence.

Team Leadership & Support

  • Assist with staffing schedules and daily staffing coordination.
  • Support the hiring, onboarding, training, and development of caregivers and medication technicians.
  • Monitor team performance and identify coaching opportunities.
  • Assist with ensuring required certifications, trainings, and regulatory requirements remain current.
  • Foster a positive team culture centered on respect, accountability, and service excellence.

Community Operations

  • Coordinate resident move-ins and orientation processes.
  • Maintain accurate resident charts and documentation.
  • Conduct medication cart audits and quality assurance reviews.
  • Collaborate with department leaders to ensure residents receive comprehensive services.
  • Assist with tours, resident assessments, and community outreach efforts when needed.
  • Support community operations in the absence of the Wellness Director.


Required

  • Medication Administration Certification or ability to obtain certification.
  • Minimum of two years of experience in senior living, healthcare, or related field.
  • Strong organizational, communication, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficiency in Microsoft Office and electronic documentation systems.
  • Valid driver's license and reliable transportation.

Preferred

  • Experience in Assisted Living or Memory Care.
  • Knowledge of South Carolina DHEC regulations.
  • Dementia care experience.
  • Supervisory or leadership experience.
  • CPR certification.

Physical Requirements

  • Ability to stand, walk, bend, and lift throughout the workday.
  • Ability to lift up to 40 pounds.
  • Ability to push medication carts and assist residents as needed.
