Position Overview:
Seeking an experienced Home Health RN Care Manager to plan, organize, and direct home care services for patients in their homes. The RN will manage patient caseloads efficiently while collaborating with interdisciplinary teams to achieve patient care goals and outcomes.
Requirements: ✅ Active RN license in California ✅ BLS/CPR Certification ✅ Valid Driver’s License ✅ Reliable automobile with current insurance ✅ Must be 21 years of age or older
