SUMMARY:

The BH-JI/CSP Case Manager must have basic knowledge of mental health and addiction recovery processes, services, and resources. Basic knowledge of the criminal justice/probation/parole system is beneficial. This role requires knowledge of and experience with community resources and referral systems and/or the ability to work in collaboration with community-based providers and agencies with exceptional customer service. The case manager must be able to multi-task, manage caseload of approximately 17-20 patients, and track multiple required tasks using CRM software system.

ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:
    • Conduct patient interviews, complete basic assessments, and engage patients into treatment and continuing care planning. Some interviews and assessments will take place in Houses of Correction.
    • Conduct case management service plans.
    • Some psychoeducation groups may be conducted within Barnstable County House of Correction or the Duke’s County House of Correction.
    • Prepare care plans in collaboration with patients and their identified supports.
    • Establish connections and utilize community resources to help patients develop effective recovery support systems.
    • Familiarity with regulatory requirements and utilization practices with ability to develop and manage systems to comply with industry standards.
    • If indicated and directed, accompany (including transporting) patients to community resources and other services as dictated by their service plans.
    • Work with patients to develop transportation and other resources to facilitate self-management of care and services.
    • Participate in staff supervision and in-service training programs.
    • Participate in team meetings to review patient progress.
    • Communicate regularly with referral sources, advocacy groups, community resources, Sherriff’s Departments, Probation, Parole, and Houses of Correction.
    • Maintain patient records in compliance with existing Gosnold standards.
    • Must be able to work well on a multidisciplinary team.
    • Able to fulfill outreach functions with own transportation.
    • Perform all other duties as assigned.

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.

    • Education: Bachelor’s degree in counseling, psychology, social work, nursing, or related discipline required.
    • Experience: Two (2) years of either professional or lived experience in mental health and addiction.
    • Skills: Strong interpersonal skills, strong organizational skills, self-directed, and proficient with web-based electronic health records. Must feel comfortable engaging with individuals involved in the justice system and going into various Houses of Correction.
    • Certificates, Licenses, and Registrations: Valid, clear MA driver’s license.
