Prefer two (2) years experience providing residential care to the elderly or similar experience with specialized dementia care programs. SEQUOIA SENIOR LIVING JOB DESCRIPTION The Nurse Advocate is responsible for coordinating resident care in assisted living in order to ensure the residents receive quality care in accordance with their care plan and Ponte Palmero’s care standards. They will conduct resident assessments for existing residents as well as potential new residents. They will closely manage the most high-acuity residents and communicate frequently with families in addition to visiting residents who are out of the community. Key to this role is overseeing wound care management and collaborating with In-Home Care, hospice, hospital personnel, physicians, community organizations and other health related service agencies to provide quality care to the residents. Reports to: Assisted Living Director. Ensures provision of services to the residents and ensures that the all Federal and State policies governing resident care are closely monitored and carried out.Responds to emails and voicemails first thing in the morning. Reviews “End of Shift” and “Med Tech to Med Tech” logs as requested by Assisted Living Director. As requested by Assisted Living Director, oversees resident returns from the hospital, skilled nursing facility etc. This includes assessing the resident’s immediate status, receiving all discharge instructions and requesting of medications from pharmacy and/or doctor’s office.Coordinates and monitors wound care management in concert with In-home healthcare agencies, hospice or any other organizations that provides direct wound care to resident population. As requested by Assisted Living Director, implements and maintains database for resident 602’s, list of residents with dementia diagnosis and residents who can leave property unassisted. Obtains updated 602’s annually for residents with dementia diagnosis. This list should be provided to transportation and maintenance departments on a monthly basis. As requested by Assisted Living Directed, oversee the management of resident chart binders including but not limited to making sure all 602’s are up to date (as per state regulations) binders are complete and organized and that all paperwork is in compliance with state licensing.Review outside agency documentation as requested by Assisted Living Director.Complete daily rounds and monitor resident care (showered, appearance, clothing etc.).Handles emergencies and first aid as needed.Ensures alert charting completed per policy and as requested by Assisted Living Director. As requested by Assisted Living Director, conducts and coordinates the completion of resident assessment program in accordance with current rules, regulations, guidelines and company policies that govern resident assessment; administer pre-admission and periodic assessments to all residents and monitor changes in status. Ensures that an initial resident assessment is completed prior to each resident’s move-in date and a comprehensive care plan is completed upon move-in. Provides reassessments on a regular basis. Ensures that all assessments and re-assessments are by reviewed and approved by Director prior to communicating to family and billing department.Completes billing status changes as needed and submits to BOD.Interviews potential residents and families, completes a pre-placement assessment and needs and services plan, and obtains medical records and other requirements as outlined in Title 22 for an admission to the community.Conducts interviews and observations on a continual basis of the residents to alert the Assisted Living Director, family and others of changes in the physical or emotional health of the resident. Conduct weekly service plan meeting.Ensures all relevant resident information is accurately entered into the AL Advantage software program.As requested by the Assisted Living Director, processes resident returns from the hospital, skilled nursing facility etc. This includes assessing the resident’s immediate status, receiving all discharge instructions and requesting of medications from pharmacy and/or doctor’s office. Ensures all changes to orders are processed timely with any new medications “in house”.Coordinates nursing services with other resident services to ensure the continuity of the residents’ total regimen of care.Acts as care manager and resource person for the residents and their families (especially for those residents determined by Director as “difficult”). Contacts physicians, family and others to obtain specific orders for care and follow-up.Maintains a safe and secure environment for all staff, residents and guests, following established safety standards.Performs fire and safety procedures when needed to assure resident/staff safety.Per State of California regulations, as a “mandated reporter” reports all known or suspected incidents of resident abuse of dependent adults or elders.Performs day-to-day clerical work connected with the position as necessary.Promotes and displays a spirit of teamwork in performance of daily duties through cooperative interactions with co-workers and other departments.Supports a positive and professional image through actions and appearance.Performs other duties consistent with the position as assigned by the Assisted Living Director.MINIMUM QUALIFICATIONS Minimum Age Requirement: Must be at least 18 years of age. Education: Must have a valid RN or LVN License Experience: Prefer two (2) years’ experience providing residential care to the elderly or similar experience with specialized dementia care programs. Mathematical Skills: Ability to perform calculations in support of personnel actions, budget, and other financial responsibilities. Reasoning Skills: Understand and conceptualize concepts as well as demonstrate ability to make decisions in stressful situations. Oral/Written Communication Skills Be able to explain ideas and to communicate complex ideas to a wide audience (employees, residents, family members, governmental agencies, public, etc.), both in writing and verbally. Equipment Used: Be proficient in basic office equipment (including a personal computer) for the purpose of accomplishing and maintaining a high level of job performance. Physical Effort: Must be mobile and able to perform physical requirements of the job. Good physical health verified by a health screening, including a chest x-ray or an intradermal test result not more than 6 months prior or seven (7) days after employment. Personal Characteristics: Ability to show sincere compassion towards cognitively impaired older adults. Demonstrates genuine concern for the physical and emotional needs of older people and their families. Working Conditions: Well-lighted office with appropriate ventilation and temperature levels. Other: (As required by Title 22) Employee must pass a criminal record clearance prior to beginning work in the community. Employee must possess current first aid training.
