Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team in Brooksville, FL working in a home health setting. Provide evaluations, OASIS assessments, and ongoing skilled physical therapy services while supporting patients in their homes. Applicants will be expected to complete required documentation and collaborate with interdisciplinary teams to promote patient safety and functional independence.


Minimum Qualifications


  • Master’s degree in Physical Therapy or higher from an accredited program. Bachelor’s acceptable if credentialed per state requirements.
  • Active Florida Physical Therapy License required prior to start.
  • Home health experience required.
  • Experience completing OASIS assessments, including SOC, evaluation, and recertification required.
  • Proficiency with Home Health documentation software HCHB required.
  • BLS certification from AHA or ARC may be required.
  • Ability to perform assessments and deliver care across Citrus and Hernando County coverage areas.


Preferred Qualifications


  • Previous travel or per-diem home health physical therapy experience preferred.
  • Experience managing productivity expectations in point-based systems preferred.


Assignment Details


  • Shift hours: 0800 - 1600.
  • Work schedule: Monday through Friday, M-F 8:00am–4:30pm as noted for site schedule.
  • Weekend requirement: Two Saturdays on-call or scheduled per assignment needs.
  • Coverage area: Citrus and Hernando County patient visits.
  • Mileage reimbursed between visits from first patient to last patient.

Impacting Patient Care


Provide skilled, patient-centered physical therapy in the home to support recovery, mobility, and safety. Conduct OASIS assessments and comprehensive evaluations to inform care plans and recertifications. Coordinate with nurses and other clinicians to ensure continuity of care and adherence to regulatory and documentation standards. Maintain productivity expectations while delivering quality outcomes for patients in the community.
