🩺 Position Overview:
We are seeking an experienced RN Case Manager / Care Coordinator with strong acute hospital experience in Care Coordination and Discharge Planning. The ideal candidate thrives in high-volume settings and works collaboratively to ensure safe and efficient patient transitions of care.
 

📋 Requirements:
✅ Minimum 2 Years Acute Hospital Case Management Experience
✅ Strong experience with Care Coordination & Discharge Planning
✅ Experience in fast-paced hospital environments
✅ Epic Charting Experience Required
✅ Travel Experience Preferred
✅ BLS Certification Required
 

⭐ Preferred Experience Includes:
• ICU, Med-Surg, ED, PCU/Stepdown
• LTAC/Rehab/SNF
• Behavioral Health & Home Health
• Utilization Review & InterQual Criteria
• Prior Authorizations & Medical Necessity Review
• DME Coordination & Plan of Care Management
 

🛠 Key Responsibilities:
• Coordinate patient care and discharge planning
• Assess patient needs and develop individualized care plans
• Determine medical necessity using evidence-based guidelines
• Collaborate with physicians, nursing staff, insurance providers, and families
• Coordinate post-acute services, authorizations, and resources
• Maintain compliance with CMS, HIPAA, OSHA, and Joint Commission standards
