Family Practice Physician (MD/DO)Location: Antioch, CA

Job Type: Full-Time or Part-Time

Job Summary & Responsibilities:
  • Provide comprehensive primary care services to patients of all ages in a community-focused outpatient healthcare setting.
  • Serve as the Primary Care Provider (PCP) for an assigned patient panel while delivering evidence-based, patient-centered care.
  • Conduct physical examinations, diagnose acute and chronic medical conditions, and develop individualized treatment plans.
  • Prescribe medications, order and interpret diagnostic testing, and coordinate specialty referrals when appropriate.
  • Provide preventive care services, health education, and chronic disease management.
  • Deliver care through both in-person and telehealth visits as appropriate.
  • Maintain accurate and timely EHR documentation while ensuring compliance with regulatory and organizational standards.
  • Collaborate with physicians, nurse practitioners, physician assistants, nurses, behavioral health providers, pharmacists, social workers, and other interdisciplinary team members.
  • Support quality improvement initiatives, utilization review activities, and ongoing patient care optimization.
  • Provide clinical oversight and mentorship to assigned Advanced Practice Providers when applicable.
  • Participate in chart reviews, performance improvement projects, and clinical best practice initiatives.
  • Manage an average patient volume of approximately 18-22 patients per day.
  • Provide care to a diverse patient population, including pediatric, adult, and geriatric patients.
Education, Licensure, and Experience:
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.
  • Active California Physician License required.
  • Current DEA Registration required.
  • Current BLS Certification required.
  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, Pediatrics, or Preventive Medicine preferred.
  • Experience in Primary Care, Family Medicine, Community Health, or Outpatient Practice preferred.
  • Experience utilizing Electronic Health Records (EHR) systems.
  • Strong clinical, communication, and interpersonal skills.

Schedule:
  • Flexible Full-Time or Part-Time opportunities available.
