Job Summary & Responsibilities:
- Provide comprehensive primary care services to patients of all ages in a community-focused outpatient healthcare setting.
- Serve as the Primary Care Provider (PCP) for an assigned patient panel while delivering evidence-based, patient-centered care.
- Conduct physical examinations, diagnose acute and chronic medical conditions, and develop individualized treatment plans.
- Prescribe medications, order and interpret diagnostic testing, and coordinate specialty referrals when appropriate.
- Provide preventive care services, health education, and chronic disease management.
- Deliver care through both in-person and telehealth visits as appropriate.
- Maintain accurate and timely EHR documentation while ensuring compliance with regulatory and organizational standards.
- Collaborate with physicians, nurse practitioners, physician assistants, nurses, behavioral health providers, pharmacists, social workers, and other interdisciplinary team members.
- Support quality improvement initiatives, utilization review activities, and ongoing patient care optimization.
- Provide clinical oversight and mentorship to assigned Advanced Practice Providers when applicable.
- Participate in chart reviews, performance improvement projects, and clinical best practice initiatives.
- Manage an average patient volume of approximately 18-22 patients per day.
- Provide care to a diverse patient population, including pediatric, adult, and geriatric patients.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.
- Active California Physician License required.
- Current DEA Registration required.
- Current BLS Certification required.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, Pediatrics, or Preventive Medicine preferred.
- Experience in Primary Care, Family Medicine, Community Health, or Outpatient Practice preferred.
- Experience utilizing Electronic Health Records (EHR) systems.
- Strong clinical, communication, and interpersonal skills.
