First Choice Health Centers is seeking a patient focused Community Health Data & Outreach Coordinator to join our team and help support our diverse patient community. This role is vital to the Center helping to advance the quality and continuity of patient care at First Choice Health Centers. This position is responsible for analyzing clinical and operational data to identify gaps in care, generating meaningful reports and conducting proactive patient outreach to ensure patients receive timely, appropriate and comprehensive care.

Minimum Knowledge, Skills & Abilities Required:

Bachelor’s degree required in Public Health Administration, Healthcare Management, Social Work or related field. Equivalent work experience will be considered in lieu of degree requirement.Strong analytical and critical thinking skills with the ability to interpret and communicate data clearly Advanced proficiency in Microsoft Office, particularly Excel, and working knowledge of data warehousingFamiliarity with quality improvement frameworks, HEDIS measures or UDS reporting preferredAbility to listen and convey messages in a clear, concise, and empathetic manner using the appropriate communication mediumTeam oriented with good interpersonal skills and the ability to collaborate effectively with coworkers and management teamComfortable challenging peers in a healthy, constructive wayExcellent verbal and written communication skillsAbility to interact professionally with a diverse group

 

Experience and Training:

1 – 2 years of experience in healthcare, public health or community health setting preferred1 – 2 years of experience with data reporting and analysisStrong knowledge of population health management initiatives such as Patient Centered Medical Home (PCMH) and HEDISDemonstrated ability to interpret healthcare data and effectively present highly sophisticated analysis and findingsDemonstrated ability presenting to small work groups on both technical and functional requirementsExperience with electronic health record software highly desired



Standard Job Duties:

Data Analysis and Reporting:

Perform ad hoc analytical requests and special projects assigned by managementCollect, analyze and interpret clinical and operational data to identify trends, gaps in care and opportunities for quality improvement.Generate and distribute regular reports on key performance indicators (KPIs) including HEDIS measures, UDS reporting and patient outcomes.Maintain and update dashboards and tracking tools to monitor progress toward organizational quality benchmarksCollaborate with clinical and administrative teams to ensure data integrity and accuracy within the electronic health record system.


Quality Improvement:

Identify patients with gaps in preventive care, chronic disease management, and screenings using registry and electronic health record generated reportsDevelop and implement targeted outreach strategies to address identified care gaps across assigned patient populationsMonitor closure rates for gaps in care and report progress to PI Coordinator.Collaborate with providers and care teams to support gap closureSupport the organization’s compliance with PCMH and other regulatory quality standards


Patient Outreach & Engagement

Conduct proactive outreach to patients via phone and electronic communication to address outstanding care needsTrack and document all outreach efforts and patients responses in EHRAssist in coordinating follow-up care for patients who have missed appointments or have unresolved care gaps
