Responsibilities
Deliver individualized physical therapy treatments to patients to meet therapeutic goals.
Assess patient progress and adjust treatment plans based on measurable outcomes.
Meet or exceed individual performance targets and contribute to pipeline-based goals.
Conduct initial assessments, screenings, and re-evaluations to establish care plans.
Document all encounters in electronic health records with accuracy and timeliness.
Collaborate with a multidisciplinary team to coordinate comprehensive care.
Maintain up-to-date clinical competencies through continuous learning and certification.
Demonstrate adherence to safety, compliance, and professional standards in all activities.
Educate patients and families on home exercise programs and injury prevention.
Engage in on-site operations, ensuring timely patient flow and effective utilization of resources.
Identify opportunities for service expansion within the territory to support growth.
Report outcomes and insights to supervisors to inform strategic decisions.
Experience/Qualifications
Licensed physical therapist with active state license and compliance
Deliver individualized PT plans and document progress in EHR accurately
Perform initial assessments, re-evaluations, and establish care plans
Adhere to safety, compliance, and professional standards at all times
Meet patient-care targets and support pipeline-based growth goals
Collaborate with multidisciplinary team for coordinated patient care
Maintain clinical competencies through continuous education and certification
Educate patients and families on home exercise and injury prevention
Ensure on-site operations, patient flow, and resource utilization
Identify territory opportunities for service expansion and growth
