Responsibilities
Assess and treat patients with mobility, pain, and function impairments.
Develop individualized PT plans with measurable short- and long-term goals.
Monitor patient progress and adjust therapy plans accordingly.
Document and maintain accurate patient records and outcomes.
Educate patients and families on home exercises and injury prevention.
Collaborate with multidisciplinary team to coordinate care.
Utilize evidence-based practices to inform treatment decisions.
Ensure compliance with safety, ethical, and regulatory standards.
Meet performance targets and contribute to pipeline-based planning.
Demonstrate proactive professional development and skill advancement.
Participate in on-site clinical rounds and case reviews.
Engage in performance reviews and contribute to merit-based rewards.
Experience/Qualifications
Licensed Physical Therapist with current state license.
Demonstrated ability to Assess, diagnose, and treat mobility and pain impairments.
Develop and implement individualized PT plans with measurable goals.
Accurate documentation of patient records and outcomes in EMR systems.
Apply evidence-based practices to inform treatment decisions.
Adhere to safety, ethics, and regulatory standards in practice.
Commit to ongoing professional development and skill advancement.
Educate patients and families on home exercise programs and injury prevention.
Collaborate with multidisciplinary team for coordinated care delivery.
Meet performance targets and contribute to pipeline-based planning.
Willingness to work on-site in a growth-oriented, merit-driven environment.
