Assignment Details
- Duration: 13 weeks
- Schedule: Monday–Friday
- Shift: 8:00 AM – 4:30 PM
Requirements (HARD STOPS)
- Active California RN license (or pending with activation by start date)
- Minimum 2 years Acute Care Case Management / Discharge Planning experience
- Hospital experience REQUIRED (Behavioral health experience does not qualify)
- Epic experience required
- Travel experience required (no first‑time travelers)
Responsibilities
- Perform comprehensive patient assessments and care coordination
- Lead discharge planning for acute care patients
- Conduct utilization review and ensure appropriate level of care
- Facilitate family meetings and care plan discussions
- Arrange post‑acute services including SNF, DME, and transportation
- Maintain accurate documentation using Epic EMR
