Position Overview:
Tide Health Group is seeking an experienced Speech and Language Pathologist (SLP) to provide evaluation, treatment, and rehabilitation for patients with communication, cognitive, or swallowing disorders. The SLP will develop individualized plans of care, monitor progress, and provide family and caregiver education in a rehabilitation/long-term care setting.
Key Responsibilities:
Screen, evaluate, and treat patients with speech, language, cognitive, or swallowing disorders.
Use standardized assessments and evidence-based interventions.
Develop and revise patient care plans according to clinical outcomes.
Document treatment minutes and maintain patient records per regulatory and clinical standards.
Provide consultation and counseling to patients, families, and caregivers.
Participate in patient care conferences, utilization reviews, rehabilitation meetings, and facility trainings.
Supervise and mentor CFY or student SLPs.
Adhere to ASHA and Texas State licensure standards, codes of conduct, and ethics.
Ensure compliance with DME and supply needs per regulatory standards.
Submission Requirements:
Minimum 1 year of SLP experience – [REQUIRED]
LTC or Rehab experience – [REQUIRED]
Active TX State License – [REQUIRED]
CCC-certified SLP – [REQUIRED]
Preferred: 3 years holding active CCC
One reference at time of submission accepted
Must be submitted at posted rate; higher requested rates will redirect candidate
Certifications Required:
BLS (AHA)
Licensure Requirements:
Texas SLP State License (Compact Allowed)
Additional Notes:
Facility EMR: Restore
Full-time hours in one facility; no travel between buildings
Participation in facility and company-required trainings, in-services, and conferences
