About this job
Qualifications:
Bachelor's Degree Public Health, Social Work or other related field of study required. Related experience can be accepted in lieu of degree.
2+ years of experience in community health outreach, health education, or related field required.
Experience with community-based work, patient home visits, SUD, homelessness and a complex patient population required.
Experience with Social Determinants of Health (SDOH), housing, food insecurity, transportation, utilities, employment, childcare assistance programs required.
Experience helping patients or clients apply for state/federal programs/benefits (i.e. SSDI, SSI, SNAP) and advocating for the patient with appeals required.
Knowledge of / familiarity with transportation resources (PT1) required.
Community Health Worker certification strongly preferred

Demonstrated ability to work effectively and provide advocacy for all populations and communities.
Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
Ability to work independently and as part of a team.
Basic computer skills, including Microsoft Office and database management.
Bilingual skills preferred.
Schedule and Work Model:
Full time, Monday through Friday, standard business hours.
Hybrid: Approximately 3-4 days/week in the community (including patient home visits) in the Cambridge, MA/Medford, MA area (as far as Burlington, MA / Lexington, MA) and approximately 1-2 days/week remote / work from home.
Remote workdays require stable, quiet, secure, compliant workstation using MGB issued equipment.
