Salary: $87,110/Annually - Salary is Negotiable (Employees will receive 5% below the negotiated pay rate during their probationary status.)
Responsible To: Division Director
Summary: This position provides strategic leadership and operational oversight for programs and services that support patients across the full spectrum of care specific to long term care. Management of daily operations of the long-term care facility, ensuring high quality resident care, compliance with Tribal, State and Federal regulations, and assures financial stability. This role ensures seamless transitions, high-quality care coordination, and strong partnerships with internal teams and external providers. This position will oversee operations in accordance with all applicable laws, regulations, policies and procedures for operation of a skilled nursing facility. Continuation of this position is contingent upon funding allocations.

- Current State of WI Nursing Home Administer License
- Bachelor’s Degree in Health Care Management, Administration or an Ancillary Profession Bachelor’s degree in Social Work, Healthcare Administration, Nursing or related field required; five (5) years’ experience in supervision; three (3) years management experience in long term care. A combination of education and experience may be considered if the State Home Administrator Licensure is valid.

- Employees are required to have proof of seasonal influenza vaccination; medical or religious exemption may be requested upon hire.
- Background information disclosure (BID) form.
- Three (3) letters of reference,
- (a) one from current supervisor,
- (b) one from past supervisor,
- (c) professional reference.
