Home Health Physical Therapist - $2,500 SIGN ON BONUS

Job Description:

As a Physical Therapist, you'll provide high-quality home-based care and help patients achieve optimal outcomes. Responsibilities include delivering direct patient care, evaluating functional needs, designing personalized care plans, and documenting all treatment and progress accurately. You will collaborate with other specialists, direct support staff, and maintain compliance with physical therapy standards of care.

• Promote Elara Caring's mission and ensure quality of care.
• Provide direct patient care and evaluate function and outcomes.
• Design and implement individualized plans of care.
• Document evaluations, treatment goals, and care plans accurately.
• Provide leadership and promote physical therapy within the community.
• Potentially work irregular hours and perform physically demanding tasks, including lifting patients.

Requirements:

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license in the state of practice.
• Minimum of one (1) year experience in a healthcare facility.
• Able and willing to travel within designated coverage area (up to 50%).
• Valid drivers license, dependable vehicle, and auto insurance.
