LTC Therapy is seeking a Physical Therapist Assistant (PTA) to join our rehabilitation team at Blackstone Valley Health & Rehabilitation. This position offers the opportunity to work with residents in a skilled nursing and post-acute care setting, supporting recovery, mobility, and functional independence.

Position Responsibilities:

• Provide physical therapy treatments under the direction of the supervising Physical Therapist
• Assist residents with therapeutic exercises, gait training, transfer training, balance activities, and mobility programs
• Observe resident performance and communicate progress and concerns to the Physical Therapist
• Encourage resident participation and support achievement of individualized therapy goals
• Collaborate with nursing and interdisciplinary team members to promote continuity of care
• Complete accurate and timely documentation in accordance with facility standards

Qualifications:

• Active Physical Therapist Assistant license required
• Strong communication, teamwork, and organizational skills
• Skilled nursing experience preferred but not required
