Physical Therapist (PT) Home Health PRN

Job Description:

As a Physical Therapist, you will provide high-quality care to patients in their home setting by evaluating functional needs and outcomes, consulting with other specialists, and adjusting care plans to ensure optimal outcomes.

• Promote Elara Carings philosophy, mission statement, and administrative policies to ensure quality of care.
• Provide direct patient care and evaluate functional needs and outcomes.
• Design and implement individualized plans of care based on thorough assessments.
• Conduct therapy practice within defined standards of care and direct support staff to achieve goals.
• Document evaluations, treatment goals, and plans, maintaining prescriptions and signatures.
• Provide direction and leadership to staff and act as a resource to promote physical therapy in the community.

Qualifications:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Valid drivers license, dependable vehicle, and current auto insurance.
• Able and willing to travel within branch/office coverage area (up to 50%).
• Ability to frequently lift, push, pull, and support up to 50 pounds.
