Physical Therapist PT Home Health

Responsibilities:

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements plans of care based on comprehensive assessment and patient participation.
• Promotes Elara Carings mission and policies to ensure quality of care.
• Directs assistance of support staff to achieve patient goals and maintains accurate documentation.
• Provides leadership to staff and promotes physical therapy in the community.
• Works in a physically demanding environment, including handling and transferring patients.

Qualifications:

• Graduate of an American Physical Therapy Association (APTA) approved program.
• Current, unrestricted Physical Therapist (PT) license in the state of practice.
• Minimum of one year experience in a healthcare facility.
• Valid drivers license, reliable vehicle, and current auto insurance.
• Able and willing to travel within coverage area (up to 50%).
