Physical Therapist PT Home Health

Responsibilities:

• Promotes Elara Carings philosophy, mission statement, and administrative policies to ensure quality of care.
• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists to adjust care plans for optimal patient outcomes.
• Designs and implements a plan of care for patients based on assessments and incorporates family participation when appropriate.
• Conducts therapy within established standards of care and leads support staff to achieve patients functional goals.
• Documents evaluations, treatment goals, and plans, maintaining current prescriptions and signatures.
• Provides direction and leadership to staff and acts as a resource to promote physical therapy within the community.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Ability and willingness to travel within the assigned coverage area (up to 50%).
• Valid drivers license, dependable vehicle, and current auto insurance.
• Ability to lift, push, pull, and support up to 50 pounds, including patient handling and equipment movement.
