Home Health Physical Therapist - $15K SIGN ON BONUS

Responsibilities:

• Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
• Design and implement individualized plans of care for patients based on thorough assessments.
• Conduct therapy practice within defined standards of care and maintain proper documentation.
• Direct and support staff to achieve patients functional goals.
• Promote the organizations mission and quality care standards.

Qualifications:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must have a dependable vehicle, valid drivers license, and current auto insurance.
• Ability and willingness to travel up to 50% within the coverage area.
