Responsibilities
- Lead, guide, and direct the delivery of physical therapy services in the facility in accordance with local, state, and federal regulations, standards, and facility policies.
- Evaluate posture, gait, balance, coordination, and strength to develop a treatment plan aimed at restoring mobility and independence.
- Implement treatment plans and adjust interventions to help residents regain independence with daily tasks (e.g., chair transfers, stair negotiation) and improve overall function.
- Document patient progress and communicate findings with the care team to inform discharge planning and ongoing care.
- Ensure compliance with regulatory requirements and facility policies, including appropriate therapy documentation and quality standards.

Requirements
- Active Missouri physical therapy license, unrestricted.
- 1 year of physical therapy experience (required); LTC or rehab experience (required).
- Valid driver’s license in the state of employment.
- Submission documents: work history/resume; skills checklist completed within 1 year of submission; references; license verification; certifications.
- Knowledge of skilled nursing home regulations, procedures, laws, and guidelines pertaining to long-term care.
- Ability to work in a long-term care setting and fulfill daytime 8-hour shifts.
