Physical Therapist

Job Description:

As a Physical Therapist with Elara Caring, you will provide direct patient care, evaluate functional needs, and design individualized treatment plans to promote independence and improve quality of life in the home setting. Responsibilities include collaborating with families and interdisciplinary teams, documenting evaluations and progress, and serving as a resource for physical therapy in the community.

Responsibilities:

• Champion Elara Carings mission of delivering the Right Care, at the Right Time, in the Right Place.
• Conduct physical therapy evaluations and develop patient-specific care plans.
• Provide direct treatment and education to patients and caregivers.
• Coordinate care with interdisciplinary team members to achieve patient goals.
• Document all patient interactions, progress, and outcomes accurately.
• Supervise and provide guidance to support staff as assigned.

Qualifications:

• Graduate of an APTA-approved Physical Therapy program.
• Current, unrestricted Physical Therapist license in Rhode Island.
• Minimum 1 year of healthcare experience preferred.
• Ability to lift 50100 lbs and perform physical job duties as needed.
• Reliable transportation, valid drivers license, and auto insurance.
• Willingness to travel within the Providence/East Bay area (approximately 50%).
