We are seeing an Occupational Therapist to conduct home health visits for our Medicare and Veteran (VA) patients.
Conducting safety evaluations and recommending adaptive equipment for assistance with daily living. (ADLs)

- Meet with patients from many different backgrounds and compassionately discuss their health concerns and therapy needs
- Conduct evaluations to determine a starting point for each patient
- Create a therapy plan specific to each patient that will help them meet their mobility goals
- Work closely with other members of each patients care team to provide adequate follow-up care
- Maintain excellent records of patient therapy and progress
- Must hold the required state license for occupational therapy
- Strong communication, interpersonal skills with impeccable customer service style follow up
- Excellent organizational skills
- Good attention to detail
- The ability to stay positive and compassionate over a long period of time with patients
