Physical Therapist (PT) Home Health

Job Description:

At Elara Caring, our Physical Therapists provide high-quality care in a home health setting. Responsibilities include promoting Elara Carings mission and philosophy, providing direct patient care, evaluating functional needs and outcomes, and consulting with other specialists to develop optimal care plans.

• Design and implement individualized plans of care based on assessments and patient goals.
• Conduct therapy within defined standards of care, documenting evaluations, treatment goals, and progress updates.
• Provide leadership and direction to support staff, acting as a resource to promote physical therapy in the community.
• Ensure documentation and prescriptions are properly maintained and signed.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Ability and willingness to travel within branch/office coverage area (approximately 50%).
• Dependable vehicle, valid drivers license, and current auto insurance.
• Ability to lift, push, pull, and support up to 50 pounds.
