Physical Therapist PT Home Health PRN

As a Physical Therapist, youll make a meaningful difference delivering compassionate care and developing individualized treatment plans to improve patient outcomes.

• Promote Elara Carings philosophy, mission, and administrative policies to ensure quality of care.
• Provide direct patient care, evaluate functional needs and outcomes, and adjust care plans accordingly.
• Design and implement individualized plans of care based on thorough assessments.
• Conduct therapy practice within defined standards of care.
• Direct support staff to meet patient functional goals.
• Document evaluations, treatments, and progress; maintain accurate records and prescriptions.
• Lead and mentor staff and promote physical therapy in the community.
• Physically demanding role requiring lifting, repositioning, and transferring patients.
• May include irregular hours and on-call work.

Requirements:

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license in the state of practice.
• Minimum one year of experience in a healthcare facility.
• Ability and willingness to travel within coverage area (up to 50%).
• Valid drivers license, dependable vehicle, and current auto insurance.
• Ability to lift or support up to 50 pounds and handle physical demands of the role.
