Position Summary
The Physical Therapist (PT) is responsible for providing physical therapy services to home health and hospice patients in accordance with physician orders, agency policies, state regulations, and professional standards of practice. The PT evaluates patients, develops individualized treatment plans, implements therapeutic interventions, and educates patients and caregivers to promote optimal functional independence, comfort, safety, and quality of life.
This position is offered on a PRN (as-needed) or Independent Contractor basis and requires travel to patient homes within the agency's service area.

Essential Duties and Responsibilities
Patient Assessment and Evaluation
- Perform comprehensive physical therapy evaluations of patients in their homes.
- Assess functional limitations, mobility status, strength, balance, coordination, pain levels, and rehabilitation needs.
- Identify patient goals and develop individualized treatment plans.
- Evaluate home environments for safety and accessibility.
- Develop and implement appropriate physical therapy treatment programs based on physician orders and patient needs.
- Provide therapeutic exercises, gait training, transfer training, balance activities, strengthening exercises, and mobility interventions.
- Utilize evidence-based therapeutic techniques to improve patient function and independence.
- Modify treatment plans as patient conditions change.
- Educate patients and family members regarding disease processes, treatment plans, home exercise programs, safety precautions, and fall prevention.
- Instruct caregivers on proper patient handling, transfers, positioning, and mobility assistance techniques.
- Promote patient self-management and independence whenever possible.
- Complete all clinical documentation accurately and timely in accordance with agency policies and regulatory requirements.
- Document evaluations, treatment plans, progress notes, reassessments, and discharge summaries.
- Participate in care conferences and interdisciplinary team meetings as required.
- Maintain compliance with Medicare, Medicaid, state, federal, and accreditation standards.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and other healthcare professionals.
- Monitor patient progress and communicate significant findings to the physician and clinical supervisor.
- Recommend durable medical equipment and assistive devices when appropriate.
- Participate in discharge planning and continuity of care activities.
- Provide physical therapy interventions focused on comfort, safety, symptom management, and quality of life.
- Assist patients in maintaining the highest possible level of functional independence and comfort.
- Educate families regarding safe mobility, positioning, transfers, and fall prevention.
- Collaborate with the hospice interdisciplinary team to support patient-centered end-of-life care.
- Maintain current professional knowledge through continuing education.
- Adhere to agency policies, procedures, ethical standards, and confidentiality requirements.
- Perform other duties as assigned within the scope of practice.
Education
- Graduate of an accredited Physical Therapy program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
- Doctor of Physical Therapy (DPT) degree preferred.
- Bachelor's or Master's degree in Physical Therapy accepted if eligible for licensure.
- Current and unrestricted Physical Therapist (PT) license in the State of Texas.
- Current CPR/BLS Certification required.
- Valid Texas driver's license.
- Current automobile insurance as required by state law.
- Minimum one (1) year of clinical experience as a Physical Therapist preferred.
- Home Health experience preferred.
- Hospice experience preferred.
- Experience with geriatric patients and rehabilitation services strongly preferred.
- Knowledge of physical therapy principles, techniques, and standards of practice.
- Ability to assess, plan, implement, and evaluate patient care effectively.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time efficiently.
- Knowledge of Medicare Conditions of Participation and home health regulations preferred.
- Ability to maintain accurate and timely documentation.
- Ability to establish and maintain effective working relationships with patients, families, physicians, and healthcare team members.
- Ability to stand, walk, bend, lift, push, and pull during patient care activities.
- Ability to lift and transfer patients with or without assistance as appropriate.
- Ability to travel to patient homes within the agency service area.
- Ability to use standard clinical equipment and electronic medical record systems.
- Patient homes, assisted living facilities, nursing facilities, and hospice settings.
- Exposure to varying environmental conditions while traveling between patient locations.
- Frequent local travel required.
- PRN (As Needed) or Independent Contractor.
- Flexible schedule based on patient census and agency needs.
- Compensation per visit or according to contractual agreement.
