Physical Therapist PT Home Health

As a Physical Therapist, You Will:

• Promote Elara Carings philosophy, mission, and administrative policies to ensure high-quality patient care.
• Provide direct patient care, evaluate functional needs and outcomes, and collaborate with interdisciplinary team members.
• Review patient progress and adjust care plans to ensure optimal outcomes.
• Develop and implement individualized plans of care based on comprehensive assessments.
• Deliver therapy services within established standards of care.
• Direct the assistance of support staff to help patients achieve functional goals.
• Maintain accurate documentation including evaluations, treatment goals, care plans, and physician orders.
• Provide leadership and serve as a resource to staff while promoting physical therapy services within the community.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted Physical Therapist license valid for the state of practice.
• Minimum one (1) year of experience in a healthcare setting.
• Ability and willingness to travel within the branch/office coverage area.
• Dependable vehicle, valid drivers license, and current auto insurance in accordance with state laws.
• Ability to travel approximately 50%.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
