LTC Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team at Blackstone Valley Health & Rehabilitation. This position offers the opportunity to work with residents in a skilled nursing setting while helping them improve mobility, strength, balance, and overall quality of life.

Position Responsibilities:

• Perform physical therapy evaluations and develop individualized treatment plans based on resident needs and goals
• Provide therapy services focused on mobility, gait training, transfers, balance, strengthening, endurance, and pain management
• Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to coordinate resident care
• Monitor resident progress and modify treatment plans as appropriate
• Complete accurate and timely documentation in accordance with facility and regulatory standards

Qualifications:

• Active Physical Therapist license required
• Strong clinical reasoning and communication skills
• Skilled nursing experience preferred but not required
