Physical Therapist PTA Home Health PRN

As a Physical Therapist Assistant, you will make a difference by providing compassionate and dedicated therapy services to patients in the home setting, supervised by a Physical Therapist.

• Promotes Elara Carings philosophy, mission, and administrative policies to ensure quality of care.
• Performs direct physical therapy services per Plan of Care certified by the physician.
• Reports patient status changes promptly to the supervising Physical Therapist.
• Adheres to State Practice Act Guidelines.
• Verifies the Plan of Care before each visit and provides therapy accordingly.
• Incorporates patient goals into therapy care and instructs patients or families on prosthetic/assistive device use.
• Educates patients and caregivers on the home program.
• Works in physically demanding environments, lifting and transferring patients as needed.
• May work irregular or on-call hours when applicable.

Requirements:

• Graduate of an accredited Physical Therapy Assistant program approved by the American Therapy Association.
• Current, unrestricted PTA license valid in the state of employment.
• Minimum of 6 months PTA experience preferred.
• Dependable vehicle, valid drivers license, and auto insurance required.
• Willing to travel up to 50% within the assigned area.
• Ability to lift up to 50 pounds and assist in patient transfers.
