Job Description
A growing healthcare organization in Metro Detroit is seeking a Licensed Master Social Worker (LMSW) to join an integrated behavioral health team. This role operates within a collaborative care model that embeds behavioral health services directly within primary care, allowing clinicians to work closely with physicians, care managers, and interdisciplinary teams.
This is a structured outpatient-only position with no hospital call or inpatient responsibilities, allowing providers to focus on delivering high-quality mental health care in a stable clinical environment.
The position offers a hybrid schedule combining in-clinic and telehealth services, with manageable patient volume to support quality patient care.
Outpatient Behavioral Health Services
Conduct comprehensive mental health assessments and psychosocial evaluations.
Develop and implement individualized treatment plans.
Provide outpatient therapy services to adults and/or children.
Deliver crisis intervention and behavioral stabilization when necessary.
Maintain a structured caseload averaging one patient per hour to ensure quality and continuity of care.
Work closely with Primary Care Providers (PCPs) within an integrated care model.
Collaborate with care managers and interdisciplinary clinical teams.
Assist patients with accessing community resources and behavioral health support services.
Maintain accurate and timely clinical documentation, including progress notes and treatment plans.
Monitor patient progress and adjust interventions as needed.
Participate in discharge planning and prepare case summaries when appropriate.
Requirements
Required
Master’s Degree in Social Work, Counseling, or a related behavioral health field.
Active LMSW license in the State of Michigan.
Minimum 3 years of experience in outpatient behavioral health settings.
Experience conducting assessments, treatment planning, therapy, and crisis intervention.
Experience working within integrated behavioral health or collaborative care models.
Comfortable coordinating care alongside primary care and medication-management providers.
Strong interest in team-based care and working with vulnerable or underserved populations.
