American Consultants is seeking a travel nurse RN Acute Care Case Management for a travel nursing job in Apple Valley, California.

Location: Apple Valley, California
Schedule: Day shift, 5x8-Hour (08:00 - 16:30). Patient ratios: 1:25 weekdays, 1:45 weekends. Every other weekend rotation required. 40 hours of orientation provided.
Highlights:- Acute care case management position in a dynamic healthcare environment
- Work with diverse patient populations including neonates, adolescents, adults, and geriatrics
- Collaborate with interdisciplinary team including Physical Therapy and Social Services
- Professional dress code: business attire or scrubs
- Epic charting system utilized for documentation
- Six-week assignment with comprehensive orientation
- Active California RN license required by start date (pending license accepted during application process)
- Minimum 2 years of acute care case management or discharge planning experience in a hospital setting
- Prior travel nursing experience required; experienced travelers only
- Epic charting system proficiency required
- Core competencies: care coordination, discharge planning, utilization review, assessing patients, and organizing family meetings
- Experience with HMOs, IPAs, arranging DME, SNF placement, and ambulance transport
- Knowledge of community resources including foster care, substance abuse treatment, homelessness services, mental health crisis intervention, advance directives, and protective services
- Ability to work independently and manage caseloads up to 25 patients
- Willingness to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- Two professional references covering one year within the last three years
