Job Opportunity – Registered Nurse, Case Management – Apple Valley, CA 92307

Start Date: July 27, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), with every other weekend required to meet department needs. Patient ratios: 1:25 on weekdays, 1:45 on weekends. Case load up to 25 patients daily.

Highlights:
  • Acute care facility supporting diverse patient populations including neonates, adolescents, adults, and geriatrics
  • 42-day assignment with 40 hours of orientation provided
  • Collaborative environment with interdisciplinary support including Physical Therapy and Social Services
  • Epic charting system utilized for computerized documentation
  • Professional dress code: business attire or scrubs accepted
  • Up to 7 days of requested time off available
Requirements:
  • Active California RN license (pending license accepted, but must be active by start date)
  • Minimum 2 years of acute care case management and discharge planning experience in a hospital setting
  • Previous travel nursing experience required; seasoned travelers only
  • Epic charting experience required
  • Experience with patient assessment, care coordination, discharge planning, and utilization review
  • Competency in arranging durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport independently
  • Knowledge of HMOs, IPAs, and regulatory compliance including CMS guidelines, DRG systems, and Joint Commission standards
  • Familiarity with complex social situations including foster care, substance abuse treatment, homelessness resources, mental health crisis intervention, and advance directives
  • Ability to organize and participate in family meetings and treatment plan development
  • Must provide 2 professional references covering 1 year within the last 3 years
