Job Requirements\n

• Minimum 1 year of Registered Nurse experience required; home health RN experience strongly preferred

• Current RN license in good standing

• Associate degree in nursing or higher

• Professional certification required for the role

• Experience with OASIS assessments preferred
\n
• Strong clinical judgment, communication, and time management skills
\n
• Ability to travel within assigned coverage areas in the Fort Smith, AR region
\n

Responsibilities\n

• Provide skilled nursing care to patients in the home health setting

• Complete field nursing visits and comprehensive OASIS assessments
\n
• Document patient care accurately and in a timely manner
\n
• Coordinate care with the interdisciplinary team to support quality patient outcomes

• Manage scheduled patient visits across Fort Smith and surrounding coverage areas
\n
• Maintain productivity standards while delivering compassionate, patient-focused care
\n
• Participate in rotating weekend and weekday coverage as assigned
